does it matter to you if guest books for 1 but arrives as 2?

I have a rental with max 3 guests and have a guest right now who booked for 1 but came with a second person. I'm totally fine with this, as its not a capacity issue and I do not charge extra.

QUESTION: Is there any particular reason or risks to ask him to alter his reservation?

In the case of 2 instead of 1 which is within reason I have had them alter the reservation to reflect the number of guests and explained it was for insurance purposes and did not change their costs.

I would say only alter if you want to charge especially if they seem like they are going to be good guests.

Only if the number exceeds the maximum of people advertised for.

The “right” answer is to not allow unregistered guests. But these decisions require analysis.

My vacation home hosts—and is licensed for—6 guests. If a group of 4 book and 6 people show up, I’m not going to say anything. I’m willing to accept the risk because it is minimal. But if 10 people show up, I’ll say something because the liability is too high for me to accept, and on top of that my county has occupancy limits based on the # of rooms.

Do you have any indication the additional guest is staying? Sometimes when I travel with friends I do visit their hotels, airbnb etc. with no intention of staying behind 15 minutes.

I believe they are a couple. They came together with suitcases/bags.
